If you would like to raise any concerns, complaints and/or compliments about any aspect of the service provided by a member(s) of the SSAFA Community Healthcare team based in British Forces Cyprus, please complete the form below.
If you would like to make a complaint, you will need the following information so that we can help you:
- Know the date on which the problem occurred, and any other dates that are relevant to your situation.
- Provide us with your contact information, including an email address if you have one.
We always try to resolve issues as quickly as possible and will ask an appropriate member of the Team to contact you within three working days to talk through, and hopefully resolve, your concerns.
